Client Care Coordinator

We are seeking a Care Coordinator to support our clients and caregivers by ensuring care plans are executed at a high level. This role is ideal for someone who is organized, proactive, and enjoys building relationships with both clients and caregivers.
Key Responsibilities
- Complete in home client visits to convert prospects and sign them up for services
- Serve as a primary point of contact for clients and families
- Conduct in-person or virtual supervisory visits
- Conduct regular client check-ins and ensure satisfaction
- Support caregiver onboarding to new cases
- Monitor care quality and address concerns quickly
- Coach caregivers on client care tactics
- Partner with scheduling to ensure strong caregiver-client matches
- Document all client interactions in our management tool
- Participate in monthly on-call rotation'
- Field work with clients across DuPage, Cook, and Will counties
What We’re Looking For
- Strong communication and interpersonal skills
- Organized and detail-oriented
- Able to problem-solve and stay calm under pressure
- Comfortable working in a fast-paced, service environment
- Prior home care or healthcare experience preferred
Compensation
- $22 – $27 per hour, based on experience
